High ESR: What a Raised Erythrocyte Sedimentation Rate Actually Indicates

ESR came back at 45. Or 62. Or 80. The reference range shows 20 for men and 30 for women as the upper limit (values vary by method and age), and your result is flagged. ESR is one of the oldest blood tests in use - and one of the most non-specific. Understanding what it can and cannot tell you prevents both excessive anxiety and missed follow-up.


What ESR Measures

ESR (Erythrocyte Sedimentation Rate) measures how fast red blood cells settle to the bottom of a test tube over one hour. When inflammation is present, proteins called acute-phase reactants (fibrinogen, immunoglobulins) coat red cells and cause them to clump and settle faster.

ESR is therefore an indirect measure of inflammation - it reflects the presence of inflammatory proteins in the blood, not the source or cause.


Reference Ranges

ESR reference ranges differ by age, sex, and measurement method (Westergren method is standard):

Age and Sex Upper Normal ESR
Men below 50 Up to 15 mm/hr
Men above 50 Up to 20 mm/hr
Women below 50 Up to 20 mm/hr
Women above 50 Up to 30 mm/hr

A rough rule of thumb: ESR upper limit = (age/2) for men, (age + 10)/2 for women.

ESR values up to twice the upper limit are mild; 2-5 times is moderate; above 5 times the upper limit is markedly elevated.


What Different ESR Values Mean

ESR Mildly Elevated (1.5-2x ULN)

Very non-specific. Many benign conditions cause mild ESR elevation:

  • Any recent infection (including mild viral illness) - ESR can stay elevated for weeks after an infection resolves
  • Anaemia - low haemoglobin causes red cells to settle faster
  • Obesity
  • Pregnancy
  • Older age

A mildly elevated ESR in an otherwise well person requires context rather than immediate investigation.

ESR Moderately Elevated (2-5x ULN: roughly 40-80 mm/hr)

More likely to indicate active inflammatory or infectious disease:

  • Active infection (bacterial more than viral)
  • Autoimmune conditions (rheumatoid arthritis, lupus, vasculitis)
  • Inflammatory bowel disease
  • TB (tuberculosis) - ESR classically elevated in active TB
  • Kidney disease
  • Chronic liver disease

ESR Markedly Elevated (Above 5x ULN: >100-120 mm/hr)

An ESR above 100 mm/hr is sometimes called "pathological ESR" and warrants systematic investigation:

  • Temporal arteritis (Giant Cell Arteritis): ESR consistently above 100; associated with new headache, jaw pain, or visual symptoms in people over 50
  • Multiple myeloma: Plasma cell cancer; ESR classically very high because the abnormal immunoglobulins heavily coat red cells
  • Serious bacterial infection: Osteomyelitis, endocarditis, abscesses
  • Active TB: Particularly pulmonary or disseminated
  • Autoimmune flares: Lupus, vasculitis

ESR vs CRP: Which Is More Useful?

ESR CRP
Response time Slow - rises over days, falls over weeks Fast - rises within hours, falls within days
Specificity Lower Slightly higher
Best use Monitoring chronic disease activity Detecting acute infection/inflammation
When they diverge ESR high, CRP normal: chronic, low-grade (TB, myeloma, autoimmune) ESR normal, CRP high: very acute early inflammation

For most acute illnesses, CRP is more useful than ESR. For monitoring chronic conditions like rheumatoid arthritis or TB treatment response, ESR is useful because of its slower dynamics.


When Elevated ESR Warrants Follow-Up

A single mildly elevated ESR in an otherwise healthy person is usually observed and repeated in 4-6 weeks. If it remains elevated without a clear explanation:

  1. Full blood count - anaemia, WBC abnormalities
  2. CRP - to see if acute inflammation is present
  3. Urine routine - kidney involvement
  4. Serum protein electrophoresis (SPEP) - to exclude myeloma if ESR is very high, particularly in older adults
  5. Chest X-ray - to exclude TB or lung disease
  6. Autoimmune markers (ANA, anti-dsDNA, RF) - if autoimmune disease is suspected
