SGPT vs SGOT: What the Difference Between These Liver Tests Tells You

Your liver test shows SGPT 85 and SGOT 48. Or SGPT 55 and SGOT 90. The individual values are one thing, but the relationship between them - specifically which one is higher and by how much - is often more informative than either value alone. This is the De Ritis ratio, and it is one of the most useful pattern-reading tools in liver interpretation.


SGPT and SGOT: A Quick Comparison

The key difference: SGPT is more liver-specific. When only SGPT is elevated and SGOT is normal, the problem is almost certainly in the liver. When only SGOT is elevated, a muscle or cardiac source is equally plausible.


The De Ritis Ratio: SGOT/SGPT

The De Ritis ratio is calculated as SGOT divided by SGPT. It was described by Fernando De Ritis in the 1950s and remains clinically useful today.

Reading Common Patterns

SGPT Higher Than SGOT (ratio below 1)

The most common pattern in routine health checkups. When SGPT is 70 and SGOT is 45, NAFLD or chronic hepatitis is the most likely cause. The liver is the source. Fat accumulation, viral hepatitis, or autoimmune hepatitis all typically show this pattern.

Key question: Is there an ultrasound showing fatty liver? Has viral hepatitis been excluded?

SGOT Higher Than SGPT (ratio above 2)

Unusual in pure liver disease. When SGOT is 80 and SGPT is 38, consider:

  • Recent intense exercise: Skeletal muscle is rich in AST. A heavy gym session or physically demanding work in the 48 hours before the test can raise SGOT substantially while leaving SGPT normal.
  • Alcohol-related liver damage: Classic pattern. GGT is also typically elevated with alcohol. The higher the SGOT:SGPT ratio, the more likely alcohol is involved.
  • Cardiac muscle damage: Heart attack or myocarditis releases large amounts of AST. If there are any cardiac symptoms, check troponin.
  • Hypothyroidism: Low thyroid hormone impairs muscle cell metabolism, raising AST.

Both SGPT and SGOT Elevated Together

When both are meaningfully elevated (e.g., SGPT 180 and SGOT 120), this is significant liver cell injury. The ratio still guides the differential:

  • SGPT:SGOT ratio above 1 → hepatocellular liver disease (NASH, hepatitis, autoimmune)
  • SGOT:SGPT ratio above 2 → alcoholic hepatitis (or acute hepatic ischaemia)

GGT: The Third Enzyme in the Picture

GGT (Gamma-Glutamyl Transferase) completes the liver enzyme triad:

  • Elevated GGT + elevated SGOT with high SGOT:SGPT ratio: Almost confirms alcohol as a contributing cause
  • Elevated GGT + elevated ALP: Bile duct problem (obstruction, primary biliary cholangitis)
  • Isolated elevated GGT: Alcohol, medications, fatty liver - non-specific but useful as a screening marker

Practical Summary: How to Read Your Liver Enzymes Together

  1. First, check which is higher: SGPT > SGOT points to liver; SGOT > SGPT means consider muscle/cardiac/alcohol
  2. Calculate the ratio: Above 2 strongly suggests alcohol or non-liver source
  3. Add GGT: Elevated GGT with high SGOT:SGPT ratio + alcohol history = alcoholic liver disease with high confidence
  4. Consider the absolute values: A ratio of 2 with SGPT 30 and SGOT 60 is less concerning than a ratio of 2 with SGPT 200 and SGOT 400
