Vitamin B12 200 to 300: The Borderline Low Zone Explained

Vitamin B12 came back at 245. Or 270. Or 295. The reference range on the report shows 200 or 211 as the lower limit, and your value is technically "normal" - but barely. You have read that B12 deficiency causes nerve damage, and you are wondering whether this borderline reading is something to act on.

The answer depends on the specific value, your symptoms, your diet, and what other markers are available.


The Vitamin B12 Reference Range Problem

The standard reference range for B12 (200-900 or 211-911 pg/mL at most Indian labs) has a known problem: the lower cutoff is set based on when frank deficiency symptoms (megaloblastic anaemia, overt neuropathy) appear. But functional B12 insufficiency - where B12 is not deficient enough to cause obvious symptoms but is affecting nervous system and metabolic function - can occur at levels well within the "normal" range.

Many clinicians consider B12 below 350-400 pg/mL functionally suboptimal, particularly in:

  • Vegetarians and vegans (who lack dietary B12 from animal products)
  • Older adults (whose B12 absorption declines with age)
  • People on metformin (which reduces B12 absorption over time)

What Different Values in the 200-300 Range Mean

200 to 250 Low borderline - functionally deficient in many; supplement
250 to 350 Grey zone - may be insufficient, particularly in at-risk groups

B12 200-250 pg/mL

At this level, many people - particularly vegetarians - have functional B12 insufficiency. The risk of neurological and haematological effects (though usually subtle at this stage) is real. Most clinicians would recommend supplementation.

B12 250-300 pg/mL

A genuinely grey zone. Lab says "normal" - but functional markers may suggest inadequacy. If you have any symptoms (tingling, fatigue, cognitive fog, low mood) or if you are vegetarian/vegan, supplementation is reasonable and carries virtually no risk.


Why the Symptoms Matter

Many symptoms of borderline B12 deficiency overlap with other conditions:

  • Fatigue: Common in B12 deficiency, but also iron deficiency, thyroid disease, sleep issues
  • Tingling or numbness in hands/feet: A more specific B12 symptom - the peripheral nervous system is vulnerable to B12 deficiency
  • Brain fog and difficulty concentrating: Non-specific, but often responds to B12 correction in borderline-deficient people
  • Low mood or mild depression: B12 is involved in neurotransmitter synthesis; low B12 is associated with mood disorders
  • Sore tongue or mouth ulcers: Less common but characteristic of B12 deficiency

If any neurological symptoms are present with a B12 in the 200-300 range, treat as deficiency and supplement promptly - early neurological B12 deficiency is reversible; advanced deficiency may not be.


Additional Tests to Clarify Borderline B12

When B12 is in the 200-300 zone and it is unclear whether true deficiency exists, these tests help:

  • MCV (Mean Corpuscular Volume): B12 deficiency causes macrocytic anaemia (large red cells, MCV above 100 fL). If MCV is elevated, deficiency is more likely to be clinically significant.
  • Homocysteine: Elevated homocysteine (above 15 micromol/L) is a functional marker of B12 insufficiency. It indicates B12 is insufficient for normal metabolism even when serum B12 appears borderline.
  • Methylmalonic acid (MMA): Elevated MMA is a highly sensitive marker of intracellular B12 deficiency - even more so than homocysteine. Less commonly available in India but available at major labs.

Who Is at High Risk for B12 Deficiency in India?

  • Vegetarians and vegans: B12 is found only in animal products (meat, fish, dairy, eggs). Plant-based diets provide virtually no B12. India's large vegetarian population makes B12 deficiency particularly prevalent.
  • People on metformin: Metformin reduces B12 absorption in the ileum; B12 should be monitored annually in people on long-term metformin.
  • Older adults: Gastric acid production declines with age, reducing B12 absorption from food (though not from supplements).
  • People with chronic antacid use: PPIs and H2 blockers reduce stomach acid, impairing B12 absorption.

Supplementation

B12 is water-soluble - excess is excreted, making supplementation safe at high doses.

  • Methylcobalamin is the active form and is better retained in the brain - often preferred
  • Cyanocobalamin is the most stable form and is widely used in injections
  • For levels 200-300 with symptoms or risk factors: oral 500-1000 mcg daily is effective
  • For significant deficiency: injections provide faster correction
  • Recheck B12 in 3 months after starting supplementation
